This agreement serves as a formal agreement to and between EASTCONN and the above identified party
for the following:
e Replace all existing Access Points at CES with new model (OF)
e Replace all existing Access Points at WJJMS with new model (OF)
e Replace all existing Access Points at BA with new model (OF)
e For JJIS, rewire ALL Access Points with new Cat6E Wire to designated locations (provided
in JJIS Map) and terminate both ends, one into jack for AP, another into Patch Panel
e Mount all new Aps in new location (OF)

Scheduled Date(s) of Activity: July 1, 2021 — June 30, 2022
Colchester School District agrees to pay the following fee as good and sufficient consideration for
performance of services listed above provided by EASTCONN:
Parts: (for JJIS) $6,750.00 (includes 6 Faceplate Patch Panels, 200 Modular Inserts, 10 6E Risers,
& sundries.)
Labor: $13,650.00 (21 man-days @ $650.00)
Total amount of contract for services: $20,400.00 plus applicable mileage
TERMINATING THE AGREEMENT: Either party may terminate this Agreement at any time by
giving 30-day written notice of termination. EASTCONN shall be entitled to full payment for services
performed prior to the date of termination.
Payable: Upon receipt of invoice.
